Transition Care (HLT07)
Transition Care pertains to two units:
- HLTAH407A – Assist with the rehabilitation of Clients
- HLTAH408A – Assist with the development and maintenance of client functional status
Training will assist care workers acquire specific skills to support older people to enhance or maintain functional status.
Materials utilised were developed by TAFE SA in collaboration with the Learning Development Centre.
