Transition Care (HLT07)

Transition Care pertains to two units:

  • HLTAH407A – Assist with the rehabilitation of Clients
  • HLTAH408A – Assist with the development and maintenance of client functional status

Training will assist care workers acquire specific skills to support older people to enhance or maintain functional status.

Materials utilised were developed by TAFE SA in collaboration with the Learning Development Centre.

Through the training clients will gain an understanding and knowledge of:

  • The Ageing Process
  • Values and Attitudes
  • Program Objectives
  • Agreement
  • Client Assessment
  • The Team
  • Environment for Rehabilitation/Restoration
  • Motivation, promoting confidence and safety
  • Deliver Rehabilitation / Restoration Program
  • Communication Strategies
  • Core Process to Client Centre Approach
  • Planning for discharge to alternative services
  • Supporting the client and/or family for discharge
  • Identifying outcomes and level of support
  • Case Study
